Birth Certificate
A copy of the original certificate is
made by the office. The original is returned.
Baptismal
Certificate
Unless the child was baptized at St. Therese Church, an
official baptismal certificate is required of all baptized children.
Medical Records
State
law requires each student to have a current Kentucky Immunization Certificate
and medical examination form (entry and 6th grade) on file in the school
office. Parents whose child's immunization certificate has expired will be
notified. They must have a doctor complete an updated certificate and return it
by the first day of the school year.
Dispensing of Medicine
The school staff is not permitted to dispense medicine
(including aspirin and Tylenol) to the students. If it is necessary for
students to take medication during the school day, the medicine and a signed
medication form (available in the office) must be brought to the office when the
student arrives at school. The medication will be made available to the student
at the time requested on the medication form. Prescription medication must be in
the original container. The student must be able to administer the medicine to
himself or herself under the supervision of the office staff.
Custody Information
The office needs, in writing, any
information regarding custody and visitation rights. A copy of the appropriate
section of the divorce decree should be sent to the office.

Please notify the School Office and
your child's teacher if you have a change of address or phone number during the
school year.
